Every abbreviation on the page

ODRight eye
Oculus dexter, Latin for right eye. Almost every prescription and every online order form lists the right eye first, so a row read out of order becomes two wrong eyes rather than one.
OSLeft eye
Oculus sinister, the left eye. OU means both eyes together and shows up when the two eyes share one value.
PWR / SPH / DPower
The spherical correction in diopters. A minus sign means nearsighted correction and a plus sign means farsighted correction. The sign is part of the number, not decoration.
BCBase curve
The curvature of the back of the lens in millimeters, usually somewhere between 8.0 and 9.5. Each product is made in only one or two base curves, and the rule requires this field or an equivalent designation on the prescription.
DIADiameter
The width of the lens in millimeters, usually between 13.0 and 15.0. Most products offer a single diameter, which is why it is often the same on every line of your order.
CYLCylinder
The astigmatism correction, present only on a toric prescription. It is normally written with a minus sign in soft lens practice and appears together with an axis.
AXISAxis
A whole number from 1 to 180 giving the orientation of the cylinder correction in degrees. It is meaningless without a cylinder value and never carries a plus or minus sign.
ADDAdd power
The reading addition on a multifocal prescription, written either as a number such as +2.00 or as a step such as LOW, MED, or HIGH, depending on the product.
BrandManufacturer or product
The specific lens the prescriber fitted. The rule lists the power, material or manufacturer among the required fields, and a base curve from one product does not transfer to another.

A prescription is a short document with a long list of rules

Most people meet their contact lens prescription twice: once at the front desk, and once at a checkout page eleven months later when they cannot remember what any of it means. The document itself is small, often a single card, and it is written in an abbreviation system that nobody explains because everyone in the room already knows it.

The FTC Contact Lens Rule defines a contact lens prescription as one issued in accordance with state and federal law that contains sufficient information for the complete and accurate filling of a prescription. That phrase resolves into a specific list. Here is what has to be on the paper.

Your name

The prescription is written for one patient. A seller verifying an order sends your full name and address to the prescriber, so the name on the order has to be the name on the prescription.

The examination date

The date of the eye examination. This is not the same as the issue date, and it is not the date the expiration term runs from.

The issue date and the expiration date

Two separate dates, both required. The expiration term runs from the issue date, which is when you receive the prescription at the completion of the fitting.

The prescriber's name, postal address, telephone number, and fax number

These are on the document so a seller can reach the prescriber to verify. If your copy is missing them, verification stalls before it starts.

The power, material, or manufacturer of the prescribed lens

The correction and the specific product. A power without a product is not orderable, because the same power behaves differently across materials and designs.

The base curve or appropriate designation

The curvature the prescriber fitted you in. Products are made in one or two base curves, so this field decides which line of a product page you are allowed to buy.

The diameter, when appropriate

The lens width. Most soft products are made in a single diameter, which is why it looks like boilerplate right up until you order a product that offers two.

For a private label lens, the manufacturer, the private label trade name, and any equivalent brand name

Retailer house brands are often a rebadged national brand. The rule requires the prescription to name both so you can shop the same lens anywhere it is sold.

You are entitled to a copy of this document. When a prescriber completes a contact lens fitting, the prescriber must give you the prescription whether or not you ask for it, at no extra charge, and may not condition it on buying lenses from them. If you never received one, that is worth a phone call before you start shopping rather than at the moment an order is held.

What each number is actually doing

The values on a contact lens prescription are not one kind of thing. Two of them describe the correction, two describe the physical shape of the lens, and the rest describe the product. Knowing which is which is most of what it takes to read the document confidently.

Power, the correction itself

Power, labeled PWR, SPH, or simply D, is measured in diopters. A minus value corrects nearsightedness and a plus value corrects farsightedness, and the sign is not optional shorthand you can drop. Powers step in quarter diopters through the middle of the range and often switch to half diopter steps at the strong end, because the difference a quarter diopter makes shrinks as the correction grows.

A published example makes this concrete. Alcon lists DAILIES TOTAL1 in a single base curve of 8.5 and a single diameter of 14.1, with sphere powers running from +0.50 to +6.00 in 0.25 D steps, -0.50 to -6.00 in 0.25 D steps, and -6.50 to -12.00 in 0.50 D steps. If your prescription reads -7.25 for that product, one of two things is true: the value was mistyped, or the prescriber fitted you in something else. Either way the answer is on the prescription and not in a guess.

Base curve and diameter, the fit of the lens

Base curve is the curvature of the back of the lens, expressed in millimeters and usually falling between 8.0 and 9.5. Diameter is the width of the lens, usually between 13.0 and 15.0. Neither one is a preference. Both come out of the fitting, where the prescriber puts a trial lens on your eye and watches how it sits, centers, and moves.

This is why a base curve does not travel between brands. A product is manufactured in one or two base curves and one diameter, so 8.5 in one product and 8.5 in another are two different lenses that happen to share a number. Choosing a new brand because it was cheaper and keeping the old base curve is not a substitution a buyer gets to make, and a seller verifying with your prescriber will surface it.

Cylinder and axis, for astigmatism

If your cornea is shaped more like the back of a spoon than a section of a sphere, the correction needs a direction as well as a strength. The cylinder value gives the strength and the axis gives the direction, as a whole number of degrees from 1 to 180. There is no axis 0 and no axis 200, and an axis never carries a sign. The two values are a pair. Copying one without the other produces an order nobody can fill.

Toric lenses also have to stay oriented on the eye, which is why they are built with a ballast or a thin zone that keeps them from spinning. That construction is one reason a toric lens costs more and comes in fewer parameter combinations than a spherical one from the same family.

Add power, for multifocal wearers

The add is the extra plus power for close work. Some manufacturers express it in diopters and some express it as a step: LOW, MED, or HIGH, occasionally written as a letter code. Those systems are not interchangeable across brands, and the mapping between them is a fitting decision. If your prescription says HIGH, order HIGH, and do not translate it into a number yourself.

Brand, which is a parameter too

The rule lists the power, material, or manufacturer among the fields the prescription has to carry, which tells you how the system regards the product name: not as a shopping preference but as part of the specification. Retailer house brands complicate this, and the rule handles it directly by requiring a private label prescription to name the manufacturer, the private label trade name, and the equivalent brand name where one exists. That is the field that lets you take a house brand prescription somewhere else and buy the same lens.

Your glasses prescription will not work, and here is the specific reason

This is the question that sends more people to a search box than any other part of the document, usually at the worst moment, with a checkout page open. The short answer is no. The useful answer is why.

Start with what is missing. A contact lens prescription must contain a base curve or appropriate designation, a diameter where appropriate, and the power, material, or manufacturer of the prescribed lens. A spectacle prescription contains none of those three. It has a sphere, often a cylinder and axis, sometimes an add and a prism, and a pupillary distance. There is no field on it that names a contact lens, because it was never about one.

Then look at the number that does appear on both. Even the sphere is not the same number. A spectacle lens sits roughly a centimeter in front of your eye, and a contact lens sits on it. That distance changes the effective power of the correction, and clinical practice compensates for it once the correction passes roughly four diopters. The compensation is a real calculation with a real chart behind it, done by a clinician, not something a checkout form applies to the number you type.

Finally, there is the part that never shows up in any number. Contact lenses are prescription medical devices, and the FDA is explicit that this includes decorative lenses with no corrective power at all. A contact lens fitting evaluates how a specific lens behaves on your specific eye over time. A glasses examination does not produce that information, so there is nothing to convert.

If you have a current glasses prescription and want contacts, the path is a contact lens fitting, which may include a follow-up visit after wearing trial lenses for a few days. You get your contact lens prescription at the end of that, and you are then free to buy the lenses wherever you like.

Seven transcription errors that hold up an order

A seller can either take your prescription directly or verify it with your prescriber. On the verification route, the seller sends the prescriber your name and address, the power, manufacturer, base curve or appropriate designation, and diameter where appropriate, the quantity, and the timing details. If a prescriber does not respond within eight business hours of a complete request, the prescription is treated as verified and the order can be filled. If the prescriber says the details are wrong, it cannot be.

That is the machinery your typing feeds. Every error below produces the same experience from the outside: an order that sits, then an email, then a phone call, then a week gone.

Dropping the sign

A power written as 3.00 instead of -3.00 is the single most consequential typing error on a reorder form, because +3.00 and -3.00 are opposite corrections and both exist as sellable products. Nothing downstream catches it for you if the value is otherwise valid.

Swapping base curve and diameter

The two numbers sit next to each other, both are in millimeters, and both look like a lens measurement. A base curve near 14 and a diameter near 8.6 are the same mistake read twice.

Reversing the two eyes

OD is the right eye and OS is the left. Order forms list the right eye first by convention, and a prescription photographed upside down or a spreadsheet sorted alphabetically undoes that convention quietly.

Carrying the axis without the cylinder

People copying a toric prescription in a hurry sometimes take the axis and leave the cylinder behind, or take the cylinder from the right eye line and the axis from the left. Both produce an order that a prescriber has to reject during verification.

Ordering the trial lens instead of the prescribed lens

The lens you wore home from the fitting is not always the lens on the final prescription. If a follow-up visit changed the base curve or the product, the box in your bag is out of date and the paper is right.

Assuming last year's numbers carried over

Reordering from your account history is fast and mostly correct, which is what makes it risky. A new examination can change a power by a quarter diopter or move you to a different product entirely, and the saved order will not know.

Treating a glasses prescription as a starting point

A spectacle prescription has a sphere, often a cylinder and axis, sometimes an add. It has no base curve, no diameter, and no product, and its power is measured for a lens sitting in front of your eye rather than on it. It is not a contact lens prescription and cannot be converted into one at a checkout page.

The prescription tells you two eyes are different. Your drawer should too.

There is a practical consequence to all of this that shows up after the order arrives. A large share of prescriptions carry different values for the two eyes, sometimes a quarter diopter apart and sometimes much further, and daily-disposable supply arrives as a stack of identical looking boxes with small print on one edge. The information that distinguishes a right pack from a left pack is real, printed, and genuinely hard to read at six in the morning.

People solve this with a system or they solve it by squinting. Loose boxes in a bathroom cabinet mean you cannot see your count, cannot see the foil dates, and cannot tell the two eyes apart without picking a box up and turning it toward the light.
